Editor's Note: Figures and comments are derived from inspections conducted by the Lewis County Public Health Department's Food Safety Program.


Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Other violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection.


Any establishment receiving 40 red points or any red point item repeated within an 18-month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.

American Legion Post #22, Chehalis, no violations noted.

Big T's BBQ, Centralia, 13 points total, 10 red points, noted violations:
Service workers didn't have food handler's card.
Thermometer was not working, was corrected.

Boccaata Deli, Centralia, 10 points total, 10 red points, noted violations:
Cooler was not at appropriate temperature for food storage, and was corrected.

Centralia Shell, Centralia, 30 points total, 25 red points, noted violations:
Potentially hazardous foods in hot holding area, the case was too cold. This was corrected.
Dishes had not been washed, rinsed, sanitized and air-dried.

Joy's Once Upon a Thyme, Chehalis, 35 points total, 33 red points, noted violations:
Foods found cooling at temperatures unsafe.
Foods being kept at the wrong temperature.
Due to the fact that it's a third cold holding violation for this agency, an operational plan to fix the situation must be submitted as well as a $250 fee within the next ten days.

Ken's Concession, Mossyrock, 5 points total, 5 red points, noted violations:
Cold storage temperature too high, and is the second violation.

Lakehouse Restaurant, Mossyrock, 10 points total, 10 red points, noted violations:
Holding temperatures were too high for the walk in cooler.

Food Art and Deli, Chehalis, no violations.

Santa Lucia Coffee, Centralia, no violations.

Star Tavern, Chehalis, 5 points total, 0 red points, noted violations:
The sink is directly drained to the sewer, and is cited to be corrected.

Temporary permit inspections:

Immanuel Lutheran Church, Centralia, no violations.
